    (Full title) “Errors in intravenous drug preparation between ready to administer prefilled syringes, cartridge-based syringes and traditional vial and syringe preparation: a randomized cross-over medication preparation study”

    Synopsis:

    The correct preparation of IV push medication is of critical importance in the delivery of safe patient care. However, increased workloads, higher patient volumes and staff shortages have put greater demands on nurses. Hence, the safe delivery of IV push medication may be compromised. In order to optimize patient safety, prefilled syringe products, such as BD Simplist (BDS) and cartridge-based systems such as Carpuject(CJ) are now available. A randomized cross-over simulation study was conducted to compare total time for drug preparation, preparation errors, satisfaction and preference for BDS, CJ and traditional vial and syringe preparation (TVSP). Both BDS and CJ were associated with significant reductions in mean preparation times compared to TVSP. Medication preparation errors were significantly reduced with BDS compared to both CJ and TVSP. In post study assessments, nurses were most satisfied with BDS, ranked BDS as the most preferred method and recommended BDS.

    Despite global concerns on the emergence of untreatable “superbugs,” little progress has been made in implementing operational and professional changes to control resistance development and its remarkable expense (US studies indicate resistant infection costs $18,000-$29,000 per case). This presentation summarizes fundamental issues surrounding the antimicrobial resistance threat, and how Antimicrobial Stewardship Programs (ASPs) change antimicrobial use from a game of roulette, to significant reductions in antimicrobial use, resistance, and cost, while also reducing patient morbidity and mortality.

The 2010 Affordable Care Act created a pathway for U.S. Food and Drug Administration approval of biosimilar products. This crucial step is expected to invigorate the biosimilars market, which is expected to shadow the booming biologics market, as patents for these agents begin to expire. This presentation describes how key issues regarding the clinical impact of biosimilar interchangeability are being addressed and how they are expected to impact the practice of pharmacy.
